Phosphate Binders: The Good, The Bound, and the Ugly

Overview:
This presentation will focus on the history of Phosphate Binder Medication development, dating back to the late 1960s, with an overview of each class of Phosphate Binder product manufactured, as to the designed commitment of its efficacy in the control of Hyperphosphatemia, and its ability to match the clinical assignment of Secondary Hyperparathyroidism regulation. Jerry Thornhill, DVM, DACVIM will share the strength of early Phosphate Binder deployment, as well as explore unfavorable trade-offs and advanced biomedical consequences associated with Chronic Kidney Disease.
Learning Objectives:
- Understand the development of Secondary Hyperparathyroidism in Chronic Kidney Disease (CKD), linked to the genesis of Total Body Phosphate buildup in CKD, before the recognition of Hyperphosphatemia
- Identify unfavorable biomedical consequences relative to Elevated Parathyroid Hormone.
- Assess the strength of early Phosphate Binder deployment to suppress Total Phosphate escalation in the Parathyroid Gland and attendant PTH elevation.
Jerry A. Thornhill, DVM, DACVIM
Dr. Thornhill is a practicing Board-Certified Internal Medicine Specialist at the Veterinary Specialty Center (VSC), in Buffalo Grove, Illinois (Suburb of Chicago), a multi-interdisciplinary specialty Hospital, as founding member of the Nephrology Section and Renal Therapies Unit (Hemodialysis & Plasmapheresis) Services.
Dr. Thornhill, following graduation from the University of Missouri, College of Veterinary Medicine, completed a non-funded 3-year Residency in Human Medicine in the Nephrology Department of Rush Medical College, in Chicago, before appointed to Staff Positions (Assistant & Associate Professor) at Purdue University Small Animal Hospital in the 80s & 90s.
Following the 14-year appointment at Purdue, Dr. Thornhill joined the Hines VA Hospital (Hines, IL) in Human Medicine, in the Department of Nephrology & Hypertension, as a Research Lab Animal Associate (focus on Goat Dialysis), in the early 2000s, for an 18-year part-time appointment, to combine with the development of the (currently ongoing) Nephrology Team, and Renal Therapies Unit at the VSC venue.
And, although the practice of Renal Therapies application serves as a rewarding side of Veterinary Medicine service - truly, the day-to-day practice in the clinical management of the Dog & Cat, with established Chronic Kidney Disease (CKD), mixed with the ER patient admit, with an Acute Kidney Injury (AKI), collectively, representative of the "bread & butter" Nephrology caseload, generates equal pride in the practice of general Nephrology, in the Renal Section of the VSC venue.

He is also the proud dog dad to 4 dachshunds and believes no one should be deprived of the incredible human-animal bond.
